Purpose and outcome (what “good” looks like)

When to use it (two-stage check)
- Stage 1 – Procurement: before placing the order / issuing the subcontract. This is your “should we use them?” check.
- Stage 2 – Pre-start: just before they start on site (often at pre-start meeting or first-day induction). This is your “is it still the same job and same people/plant?” check.
- Use it again if something changes: scope, programme, key people, plant, method, work area, or risk level.
Rule of thumb: the higher the risk (height, lifting, excavations, hot works, live services, public interface), the tighter your checks and the clearer your evidence should be.
Before you start (what to gather)
Have these ready so you’re not guessing or filling it in from memory:
- Scope and programme: what they’re doing, where, start date, duration, and interfaces with other trades.
- Names: their supervisor/foreman, and (if known) the operatives who will attend.
- RAMS: risk assessment and method statement that matches your job (not a generic PDF).
- Training/competence evidence (as relevant): card numbers, certificates, screenshots, or a quick list you can verify (e.g., CSCS, CPCS/NPORS, IPAF, PASMA, first aid).
- Plant and certification (as relevant): LOLER, inspection records, maintenance/PUWER arrangements, PAT approach where appropriate.
- Insurance: policy schedule/certificate and expiry dates (PL/EL; PI if design responsibility).
- Previous performance info: your own notes, defect lists, site observations, or references if you’ve not used them before.
Keep it sensible: you don’t need a mountain of paperwork for low-risk, short-duration tasks, but you do need enough to show you checked properly.
How to complete it (step-by-step)
Step 1: Fill in the project and contractor details
- Enter the correct project name/number and site address (helps filing and audits).
- Record the subcontractor’s main contact and the trade/package.
- Add planned start date and duration (capability depends on dates).
Step 2: Describe the work and interfaces (don’t skip this)
- Write a short scope in plain terms (what they will physically do).
- Note where they will work and who they could affect (other trades, public, neighbours).
- Confirm if they have any design responsibility or if they will use their own sub-subcontractors.
Why it matters: if the scope is unclear, the RAMS will be wrong and your competency checks won’t match the real risk.
Step 3: Complete the competence checks (people)
- Make sure a named supervisor/foreman is identified.
- Check the operatives have the right experience for the tasks (especially the “awkward bits” of the job).
- Where relevant, record card/cert details (numbers and expiry are enough if you’ve seen copies).
- Consider communication: how will briefings be understood on site?
Tip: use the “Evidence/notes” column to record what you actually saw (e.g., “IPAF 3a/3b, exp 06/2027, copy saved in job folder”).
Step 4: Complete the capability checks (can they deliver?)
- Confirm numbers of people available for your dates (and what happens if someone is off).
- Check supervision level matches risk and site size.
- Confirm plant/tools are suitable and available (include any hire arrangements).
- Talk through lead times and sequencing so there’s no pressure to rush or “make do”.
- Ask for similar job examples (one or two is enough if genuine).
Step 5: Health & safety basics (RAMS and site rules)
- Read the RAMS and check it’s specific: correct site, correct task steps, and real controls.
- Check it includes permits and emergency arrangements where needed (e.g., hot works, confined spaces).
- Confirm induction/briefing plan: who briefs, when, and how changes are handled.
- Confirm they know how to report near misses and accidents on your site.
Minimum standard: a method that someone could follow on site, not a generic risk list.
Step 6: Key risk experience (tick what applies and add controls)
- Tick “Yes” only if it genuinely applies to their scope.
- In the notes, write the main controls you expect to see on site (e.g., edge protection, exclusion zones, permits, service scans).
- If they say “Yes” to a high-risk area, make sure the competence and plant sections back it up.
Step 7: Plant, equipment and certification
- Only complete what’s relevant, but don’t miss key items (LOLER for lifting, inspection processes for towers/MEWPs, defect reporting).
- Record certificate references/expiry where you can.
- If plant is hired, confirm who supplies, who inspects, and who is trained to use it.
Step 8: Previous performance
- If you’ve used them before, be honest: quality, housekeeping, attitude to RAMS, reporting.
- If you haven’t used them, note what checks you did (e.g., a reference call, review of past work, recommendation source).
Step 9: Insurance and compliance
- Check the insurance types match the job and are in date.
- If they have design/spec responsibility, consider whether professional indemnity is needed.
- Note any licences/registrations that apply to the package.
Step 10: Make the decision and list actions
- Select Approved, Approved with conditions, or Not approved.
- If there are issues, write actions in plain terms: what needs doing, who will do it, and by when.
- Do not allow work to start if conditions are “must close before start”.
Step 11: Sign-off the two stages
- Stage 1 sign-off: the person appointing (or recommending appointment) signs to confirm checks were done.
- Stage 2 sign-off: the site manager/supervisor confirms nothing has changed and the subcontractor is still suitable.
- Answer the pre-start “anything changed?” question properly (people, plant, RAMS, programme, scope).
Review and approval (what to check before signing)
- Are the checks proportionate? Higher risk packages should have stronger evidence and clearer notes.
- Do RAMS match the scope and site? Watch for copied site names, missing steps, or vague controls.
- Is there a named supervisor? If not, that’s usually a stop point.
- Are plant certs in date where needed? If not, stop and resolve before start.
- Are conditions genuinely closed? Don’t sign Stage 2 based on promises.
Escalate / pause the start if you see red flags like generic RAMS, expired LOLER, unclear lifting/excavation controls, “we’ll sort it on the day”, or poor previous site behaviour.
Recordkeeping (filing, retention, audit-ready)

- programme changes create pressure (weekend work, short notice accelerations);
- there is an incident, repeated non-conformance, or poor site behaviour.

- Stage 1 (procurement): You record scope and interfaces (other trades below). You ask for RAMS covering delivery, unloading, exclusion zone, propping sequence, lifting method, and tower use.
- Competence: Subcontractor provides a named supervisor and confirms operatives have done similar installs. You note relevant training (e.g., PASMA for tower user; telehandler operator ticket if they’re using one).
- Plant/certs: You request LOLER for lifting accessories (if used) and confirm tower inspection process. You note certificate references and expiry dates.
- Decision: “Approved with conditions” because RAMS are missing a clear exclusion zone plan and do not state who inspects the tower. Action: “Update RAMS with exclusion zone and tower inspection responsibility, by Friday 12:00.”
