Purpose and outcome
This resource helps you check the Pre-Construction Information (PCI) you’ve been given and record anything that’s missing, unclear, out of date, or needs confirming. It stops the team “guessing” on site.
Good completion looks like:
- You can explain the job scope, site constraints, and key risks in plain terms.
- Known high-risk items (services, asbestos, unstable structures, ground issues, public interface) are identified early.
- Every gap is logged with a clear request sent to the Client/Principal Designer (PD) and tracked to close-out.
- Critical gaps are closed (or properly controlled) before starting the relevant work.
- You have an audit trail showing you reviewed the PCI and acted on it.
Who uses it (typical small company set-up)

When to use it (timing and triggers)
- As soon as you receive the PCI pack (tender/pre-start) — don’t wait until the week you mobilise.
- Before finalising RAMS and the programme — the gaps often change sequencing and controls.
- Before starting any higher-risk activity (e.g., breaking ground, demolition, structural alterations, hot works, work at height, isolations).
- Any time new information arrives (revised drawings, new survey, change in scope, design change).
- After a site visit where you spot differences to drawings/plans.
Minimum expectation: one initial review pre-start, then update it whenever there is a meaningful change or a gap is closed.
Before you start (what you need to have to hand)
Gather what you can first. If you don’t have it, that’s fine—log it as a gap.
- The PCI pack (and its version/date): drawings, specs, design notes, surveys, constraints, site rules.
- Scope and programme: what’s in/out, proposed start date, phases, access dates, client restrictions.
- Any existing surveys/reports: asbestos, services searches, drainage plans, topographical survey, ground investigation, contamination, structural info.
- Known site rules: permits, hot works rules, working hours, deliveries, neighbour constraints, emergency arrangements.
- Site photos / site walk notes (if you can visit): access points, overhead lines, existing structures, visible services covers, fragile areas.
- Previous job history (if you’ve worked there before): “known problems” and lessons learned.
How to complete it (step-by-step)
Step 1: Fill in the project details (so it’s traceable)
- Record project/site name, address, client, PD, PC.
- Record who did the review and the date.
- Record PCI version/date received (this matters when updates come in).
- Note the planned start date—this drives urgency for chasing gaps.
Step 2: Do the quick completeness check (first pass)
Use the checklist to quickly see what is present and what is missing. Don’t overthink it—your aim is to spot holes fast.
- Tick “Yes” only if the information is usable (current, clear, relevant to the work area).
- Tick “No” if it’s missing, too vague, out of date, or clearly doesn’t match the job.
- Use the notes column for one-line reminders (the full detail goes in the Gap Log).
Practical tip: If it’s a live services job or any groundworks, treat “no service info” as a high-priority gap immediately.
Step 3: Capture “unusual or high-risk items” early
Use this section to highlight items that can change the plan or stop the job.
- Be specific about where it is (area/room/elevation/boundary line).
- Write the risk in site terms (e.g., “unknown live cables under slab”, “fragile roof lights”, “possible asbestos insulating board”).
- Write what you need before starting (e.g., “updated asbestos refurb survey for this room”, “GPR scan + CAT & Genny + isolation plan”).
Step 4: Log gaps properly (one gap per line)
This is the working part. Each gap should be clear enough that someone can action it without a phone call.
- Ref: number each gap (01, 02, 03…).
- Topic: keep it short (e.g., “Asbestos survey”, “Electric service plan”, “Traffic management”, “Temporary works”).
- What’s missing/unclear/out of date: state exactly what you don’t have (e.g., “no isolations shown”, “survey excludes rear extension”, “utility plan dated 2019”).
- Risk if we guess: real consequence (injury, strike, collapse, fire, programme delay, enforcement).
- Request sent to: name the person/organisation (Client / PD / other) and how sent (email is best for traceability).
- Close-out: write what you received and date (e.g., “updated drawing Rev C + photos received 12/03”).
Rule of thumb: if you can’t safely plan controls from what you’ve got, it’s a gap and it goes in the log.
Step 5: Convert the output into a plan the team can use
Once you’ve logged the gaps, use them to update:
- RAMS (method changes, extra controls, sequencing, exclusion zones).
- Programme (hold points: “no dig until verified services/asbestos”).
- Permits (hot works, excavation/dig permit, lifting, confined spaces where applicable).
- Briefings/induction (make sure the key points are understood on day one).
Step 6: Close-out and re-brief
- When new info arrives, check it is the latest revision and applies to your work area.
- Update the close-out column and attach/record the evidence (drawing number, report title, revision/date).
- If it changes the method, update RAMS and re-brief the crew and any subcontractors affected.
Review and approval (sign-off expectations)
Use the sign-off section to show the review happened and the gaps were sent out.
- Reviewer (site/supervisor): signs to confirm the PCI was checked and gaps were logged honestly.
- Manager/director review: checks priority items, agrees any “no start” hold points, supports chasing the Client/PD.
- Gaps issued date: record when requests were sent (email trail helps).
- Critical gaps closed? only mark “Yes” if the items that could cause serious harm or major disruption are closed or robustly controlled (with agreement).
Escalation (when to stop and get a decision)
Escalate to management (and do not start that activity) if any of these are not resolved:
- Unverified live services in the work zone (electric, gas, water, telecoms, drainage).
- Missing/inadequate asbestos information for refurbishment/demolition areas.
- Unknown structural stability / need for temporary works (propping, sequencing, partial demo).
- Unclear public interface controls (roads/footpaths/schools, deliveries, segregation).
- Ground conditions that could affect excavations (collapse, contamination, groundwater, buried obstructions).
Recordkeeping (storage, version control, retention)
- Where to store: keep it with your project H&S file/job folder (digital preferred), and a printed copy in the site file if you use one.
- Link evidence: save the PCI pack, emails, drawings, and reports in the same folder. Name files so you can tell the revision/date quickly.
- Version control: don’t overwrite old PCI packs. Keep them as “superseded” and file the latest as “current”. Your log should reference what version you reviewed.
- Retention: keep with the job records for the same period you keep RAMS and project H&S records (follow your company policy/client requirements). If unsure, keep it for several years as it supports your audit trail.
- Audit-ready tip: a clear email chain + dated close-out entries are often more useful than long notes.
How often to review/update
- Initial review: once, as soon as the PCI arrives.
- Update: every time you receive new or revised information, and before each new phase with different risks (e.g., enabling works → demolition → groundworks → steelwork → fit-out).
- Site reality check: update after the first day on site if you find mismatches (access, neighbours, services covers, layout constraints).

- Quick check finds: drawings provided, but utility plan is dated 2020; asbestos survey is a management survey only; no info on rear yard drainage run.
- Unusual/high-risk items noted: overhead service cable at rear yard; shared alley access with neighbours; hot works needed for steel lintel.
- Gap log entries raised:
- Asbestos: request a refurbishment survey for the shop ceiling void and rear wall where the doorway will be formed.
- Services: request updated utility search/plan and confirmation of isolations; plan for CAT & Genny and trial holes before drainage works.
- Drainage: request drainage as-built or permission for CCTV survey; confirm connection point and any shared runs.
- Close-out: PD issues refurb survey (dated, covers the work area), updated drawing revision, and a note confirming working hours restrictions. You update RAMS, add a “no break-out until asbestos clear” hold point, and brief the team.
